The Chronic Care Management Improvement Act of 2026 would eliminate Medicare cost-sharing — deductibles, copays, and coinsurance — for chronic care management services starting January 1, 2027. Medicare would pay 100 percent of the lesser of the actual charge or the fee-schedule amount for these services. Chronic care management is the bundled set of coordination, planning, and monitoring services that primary care providers furnish to patients with two or more chronic conditions (such as diabetes, hypertension, or chronic obstructive pulmonary disease) — services that currently carry Medicare's standard 20-percent coinsurance plus an annual deductible. The bill amends Section 1833 of the Social Security Act (42 U.S.C. § 1395l). Democratic Rep. Suzan DelBene of Washington and Republican Rep. Mike Kelly of Pennsylvania lead the bill.
